The Admission Process
Pre-Admission Requirements
Prior to admission into Whitehall Aged Care Facility, these documents must be provided to the facility:
- Completed Enquiry Form (FORM - )
- Current ACAT Assessment Form (Original or photocopy)
Once you decide to apply for respite or permanent residency, copies of the following documents will be provided to you prior to admission:
- Resident Agreement (2 copies)
- Application for Respite or Permanent Residency Form
- Assets Declaration Form
Admission Requirements
On admission into Whitehall Aged Care Facility, the following must be provided to the Administration Officer:
- Signed Resident Agreement
- A copy of your completed Assets Declaration Form (If you have your assets assessment completed)
- Copy of Power of Attorney (if applicable)
- Copy of Enduring Guardianship (if applicable)
- Cards, if applicable (these will be returned to you once photocopied onsite):
- Medicare Card
- Safety Net Pharmaceutical Card/PBS Card
- DVA Card
- Pensioner/Concession Card
- Health Care Card
- Private Health Fund Card
- Electoral Card
- Hearing Aid Services Card
- NDSS Card (for Diabetics)
- Taxi Vouchers
